We currently complete a Comprehensive Substance Use Disorder (SUD), formerly referred to as Chemical Dependency (CD) or Rule 25, Assessment to obtain information to determine a client’s risk level in the 6 dimensions under Rule 245G. These dimensions include use history, medical history, mental health, readiness for change, relapse potential, and recovery environment. These factors assist us in determining appropriate recommendations.
Assessments can be completed on or off-site depending on client’s needs. Off-site would include jail, hospital, clinic, etc.
